Medical Records are a patient’s health history.

Hours of Operation:
Monday-Friday, 8:00 a.m. – 5:00 p.m.

Release of Information

Records are maintained for 10 years or 2 years past majority (18 year of age), whichever is greater.

A release must be completed by the patient, or legal guardian for minors.

The fees for copies of your medical records are set the by Texas State Senate and are subject to change yearly.

Birth Certificates

Birth certificates are prepared after the birth of the baby and before discharge from the hospital. The birth certificate will be filed with Hill County Courthouse.

Acknowledgements of Paternity are available for unwed mothers who would like to include the father’s information on the birth certificate. These may be completed any time prior to delivery or before discharge from the hospital.
